Change of Ownership (CHOW) Enrollment Application

A Change of Ownership (CHOW) that results in a change of Federal Employer Identification Number (EIN) terminates the Provider Participation Agreement (PPA). The new owners or providers must re-apply with the new EIN. This means submitting a new enrollment application via the Provider Enrollment Portal, which will require including the selling provider's information and signing a new PPA.

New owners or providers indicate that an enrollment application is connected to a CHOW resulting in an EIN change on the Change of Ownership panel. This panel will only appear for certain facility provider types.
 

  1. Access the Provider Enrollment Portal.

    Acknowledge the Welcome panel by clicking Continue. Complete the Request Information panel and click Continue. The user will be directed to the Change of Ownership panel.

  2. Complete the Change of Ownership Panel.

    The Change of Ownership panel automatically defaults to No as the answer to the question: Is the application due to a change of ownership where the EIN is changing?

    Update the answer to Yes. This action causes the Previous Ownership section to display. Complete all fields under the Previous Ownership section with the selling provider's information.

  3. Complete the remainder of the needed updates.

    Upon receipt of a completed CHOW enrollment application, the selling provider will receive a letter from Health First Colorado (Colorado's Medicaid program) with further instruction on the voluntary disenrollment process.

    The CHOW enrollment application cannot be processed for approval until the selling provider completes and submits a voluntary disenrollment request through the Provider Web Portal.
